To qualify for any of these phantoms your PETAP or NMAP number is REQUIRED. Once you submit your ACR Nuclear Medicine or PET accreditation applications, go to “My Applications” in the ACR accreditation database to find the corresponding NMAP and PET numbers. If you do not have these numbers, please submit a support ticket at Complete Accreditation Information: Nuclear Medicine and PET.

For any SMALL FIELD OF VIEW DEDICATED CARDIAC SPECT SYSTEMS you will probably need our Small Flangeless Jaszczak SPECT Phantom. Please place and order using our Small SPECT Phantom Order Form (this is camera specific).
